Use this form to register your event. The form has two pages:


The first page (this page) is to enter details about your organisation. ADCA collects this information to enable us to supply our event organisers with the DAW promotional packs.

The second page is to enter details about the event you plan to host. This information will be published on DAW’s National Calendar of Events upon approval by site admin.
 
Organisation: Name of the organisation hosting the DAW 2010 event.
Street Address : Please provide the street address for promotional pack delivery.
Suburb: Please provide the suburb for promotional pack delivery.
Post Code: Post Code for delivery.
Contact Name: Your DAW Event coordinator. This name will be posted on the DAW calendar.
Contact Email: DAW contact email – DAW will send out automatic confirmation of your event to this email. This email address will be posted on the DAW calendar.
Contact Phone: DAW Event Coordinator contact phone number. This number will be DAW 2010’s prime contact point. This number will be posted on the DAW calendar.
State: The State/ Territory to post your DAW material to.
